Filing out this form is the first step in securing church resources for your event. All requests will be evaluated for approval on a case-by-case basis. If approved, requestor will receive notification regarding required facility-use protocol. You will also be contacted to confirm this reservation.
Requests must be submitted 2 weeks in advance.

Technical Support Request
Events in the Sanctuary, Family Life Center, or Christian Education Building requiring use of church A/V equipment require a trained A/V operator present throughout the event.
All A/V needs must be received 2 weeks prior to event.

All drivers for your event must complete the Auto Safety Certification form, provide a copy of their license/insurance & completed training to operate church vehicles prior to reserving the vehicle.

I understand cancellations need must be made 24 hours in advance so that personnel, utilities, products are not wasted or my ministry area/outside organization may be charged.
I have obtained the proper background checks for all adults working with minors or adults with Special needs. These must be obtained 2 weeks in advance of your event.
The drivers for my event have completed the training and I have reviewed the policy for cleaning and returning the vehicles.
I have reviewed the church policies regarding acceptable facility use. (decorations, alcohol use, etc.)
I understand that supplies used outside of normal church usage may be charged to the ministry area supporting my event. For requests that include personnel assistance (musician, tech support, kitchen) an honorarium could be appropriate.
